The legal core of informed consent is information disclosure. The physician is responsible for explaining the risks, benefits, alternatives, and consequences of refusing the procedure. The nurse's role is that of a witness, responsible for confirming that the client has the capacity (competence) to sign, that the signing is voluntary, and to witness that the procedure has been completed. The nurse cannot explain surgical details on the physician's behalf, cannot sign for the client, and must not witness signing when the client is not lucid.
