When a patient with peptic ulcer disease complains of dizziness and weakness, gastrointestinal bleeding (specifically upper GI bleeding) should be strongly suspected. Bleeding reduces circulating blood volume, which in turn produces orthostatic hypotension, anemia, and tissue hypoperfusion, presenting as dizziness and weakness. The nurse must assess for melena, hematemesis, or tachycardia and other pre-shock signs, as bleeding is the most dangerous complication of ulcer disease.
