The legal responsibility for informed consent rests with the physician, who must personally explain the purpose, risks, benefits, and alternatives of the medical procedure to the client. The nurse's role is that of a witness, responsible for confirming that the client is alert, voluntary, and informed when signing the document, and verifying the signing process. The nurse should not provide explanations of the medical details, in order to avoid acting outside the scope of practice.
