A nurse is caring for a client with a Do Not Resuscitate (DNR) order. Which action is the priority if the client stops breathing?
- ACheck the client's pulse and breathing, then document findings✓
- BBegin chest compressions
- CInitiate emergency intubation
- DCall the rapid response team
A Do Not Resuscitate (DNR) order pertains only to cardiopulmonary resuscitation (CPR); that is, no rescue breathing or chest compressions are performed when the heartbeat or breathing stops. However, this does not mean that all nursing care is stopped. When the client stops breathing, the nurse should first confirm vital signs (pulse and respirations), and then document the time of death and notify the physician per policy. Comfort care and end-of-life support should continue to be provided rather than emergency resuscitation.
