Potassium chloride (KCl) is highly irritating; extravasation into subcutaneous tissue readily causes tissue necrosis and permanent injury. Before intravenous administration, the nurse must confirm that the line is patent and that blood return is present to ensure that the medication is being delivered into the vein. Potassium chloride must never be given as an IV bolus and should always be adequately diluted, because high-concentration potassium ions cause phlebitis and tissue damage.
