A transfusion reaction may occur within a short time after the transfusion is started, and the nurse must closely monitor for any changes in vital signs. Hemolytic reactions are the most dangerous, often presenting with fever, chills, lower back pain (due to hemolysis products obstructing the renal tubules), and hypotension or other signs of shock. If a reaction is suspected, the first action is to stop the transfusion immediately, maintain IV access with normal saline, and notify the provider.
