A nurse is assessing a client with cirrhosis who has ascites. Which assessment finding indicates a worsening condition?
- ADecreased abdominal girth measurement
- BDevelopment of asterixis (flapping tremor)✓
- CDaily weight gain of 1 lb
- DUrine output of 500 mL in 8 hours8 小時內尿液排出量為 500 mL
Patients with cirrhosis have impaired liver function and cannot effectively metabolize ammonia produced from protein breakdown. Asterixis (flapping tremor) is an early neurologic sign of hepatic encephalopathy, indicating that ammonia toxicity has affected the central nervous system. This means hepatic function has severely failed and, without immediate intervention, the patient may rapidly progress to coma; relative to chronic changes such as weight gain, this finding is the most emergent.
