Which of the following interventions are appropriate for a client at high risk for falls? (Select all that apply.)
- AKeep the bed in the lowest position✓
- BPlace the call light within reach✓
- CKeep the environment free of clutter✓
- DApply physical restraints to keep the client in bed
- EEnsure the client wears non-slip footwear✓
Preventing falls is a key component of inpatient care. Standard preventive measures include keeping the bed in a low position, placing the call bell within reach (for prompt summoning of help), removing floor clutter (to prevent tripping), and wearing non-slip footwear. Restraints are not the first choice for fall prevention, because they can increase the patient's anxiety, agitation, and risk of muscle atrophy; they should be used only as a last resort and only with a strict physician's order. The nurse should perform a fall risk assessment in high-risk patients and implement preventive measures.
