A nurse is teaching a client about discharge medication. Which approach is most effective for ensuring understanding?
- AAssume the client understands since they didn't ask questions
- BProvide a detailed written manual and tell them to read it提供詳細的書面手冊並告知他們閱讀
- CGive a 30-minute lecture on medication side effects
- DUse the 'teach-back' method to ask the client to repeat the instructions✓
The teach-back method is the most effective way to confirm that the patient understands teaching content; it asks the patient to explain key information in his or her own words. This helps the nurse verify whether the patient has actually absorbed the information rather than merely listened. Simply handing out a pamphlet or delivering a lengthy lecture cannot guarantee understanding, and assuming that 'no questions means comprehension' is a major risk.
