口服鐵劑(Oral iron)常見的副作用包括糞便顏色變深、呈現黑色或瀝青狀(Black, tarry stools),這是未吸收的鐵質與腸道內硫化氫結合所致,屬正常藥物反應,不代表出血。護理師需事先告知病人,以免誤判。此外,鐵劑應空腹服用以利吸收,但若引起胃部不適,可改與少量食物併服,但需避免與奶製品、制酸劑、鈣片同時服用,因這些物質會抑制鐵質吸收。
A common side effect of oral iron is darkening of the stool, which may appear black or tarry. This results from unabsorbed iron combining with hydrogen sulfide in the intestine; it is a normal medication effect and does not indicate bleeding. The nurse must inform the client in advance to prevent misinterpretation. In addition, oral iron should be taken on an empty stomach to enhance absorption, but if it causes gastric upset, it may be taken with a small amount of food; however, it should not be taken at the same time as dairy products, antacids, or calcium supplements, because these substances inhibit iron absorption.
