Hallucinations are the client's subjective sensory experiences, which are real to them. When interacting with the client, the core principle is to "accept the emotion and redirect the focus." The nurse should neither argue about the reality of the hallucination nor agree with it (that is, pretend to see or hear it), because that would undermine the nurse's professional credibility. Instead, the focus should be placed on the emotions triggered by the hallucination (such as fear or anger), and the conversation should be guided back to people and things in the real environment, helping the client return to reality testing.
