The pathologic basis of peripheral arterial disease (PAD) is atherosclerosis that narrows arteries and impairs perfusion. Clinical findings include muscle pain on exertion due to ischemia (intermittent claudication); dry, hairless skin with thickened, brittle nails from chronic hypoxia; and cool limbs with diminished pulses on palpation. Edema is characteristic of impaired venous return (such as PVD), not of arterial ischemia. Flushing is a sign of inflammation or vasodilation, opposite to the pallor or cyanosis caused by arterial occlusion.
