Lithium has a narrow therapeutic window, and serum concentrations can readily become elevated and produce toxicity. Symptoms of toxicity are most strongly associated with neurologic effects, including coarse tremors, ataxia, unsteady gait, confusion, lethargy, and severe gastrointestinal symptoms (vomiting, diarrhea). Polyuria and polydipsia are common side effects of lithium rather than signs of toxicity. The nurse should monitor intake and output (I&O) and the serum lithium level to ensure that the drug remains within the therapeutic range.
