A nurse is delegating tasks to an Assistive Personnel (AP). Which action is appropriate for the nurse to assign?
- ATeaching a client about insulin injection techniques
- BPerforming a sterile dressing change on a deep surgical incision
- CEvaluating the effectiveness of a client's pain medication
- DAssisting a client with chronic stable hemiplegia to ambulate✓
Delegation must follow the Five Rights of Delegation, one of which is the "right task." Assistive personnel (AP) may perform routine, stable tasks that do not require professional nursing judgment, such as assisting with ambulation, feeding, or hygiene. Assessment, client teaching, and complex wound care are professional nursing responsibilities (the nursing process) and must be performed by the RN.
