The social worker's scope includes linking clients to social resources, financial assistance, placement planning, and resolution of complex family issues. Difficulty paying for medications directly affects medication adherence; if the client cannot afford their medications, treatment failure and rehospitalization may follow, so a social work consultation is needed to assess available resources.
在美國，醫院社工（Hospital Social Worker）在出院規劃中扮演關鍵角色，負責處理保險、轉院至復健中心（SNF）或居家護理的資源對接。台灣則較多由出院準備服務小組（包含個管師、社工、護理師）共同協調，界線較有重疊。
