Potassium chloride is highly irritating; extravasation from the vein can cause tissue necrosis (often felt initially as burning). Hyperkalemia (peaked T waves) indicates toxicity and the infusion should be stopped. Oliguria or anuria is a warning sign of renal impairment, since potassium that cannot be excreted will rapidly accumulate to fatal levels; these three findings require immediate intervention. A serum potassium of 4.2 mEq/L and a heart rate of 62 bpm are within normal limits.
