使用物理性約束（Restraints）時，病人的安全評估為首要任務。約束帶若過緊或位置不當，可能壓迫血管與神經，導致肢體缺血、組織壞死或周邊神經損傷。因此，定期評估遠端肢體的循環（Circulation）、感覺（Sensation）與運動能力（Motion），即所謂的 CMTS 評估，是護理師最關鍵的職責。
When physical restraints are in use, safety assessment of the client is the priority. If the restraint is too tight or improperly positioned, it can compress vessels and nerves, causing limb ischemia, tissue necrosis, or peripheral nerve injury. For this reason, periodic evaluation of distal circulation, sensation, and motion — the CMS (or CMTS) assessment — is the nurse's most important responsibility.
