In chronic kidney disease (CKD), impaired renal excretion of phosphorus leads to elevated serum phosphate levels (hyperphosphatemia), resulting in calcium-phosphorus imbalance and subsequent secondary hyperparathyroidism and bone disease. Phosphate binders work by binding dietary phosphorus in the intestine to form insoluble complexes that are eliminated in the stool, thereby reducing phosphorus absorption. For maximum effect, they must therefore be taken with meals.
