A shuffling gait readily leads to an unstable center of gravity and falls. The nurse should instruct the client to lift the feet and widen the stance to improve stability. Mobility should not be restricted (option A); ambulation should be maintained. Looking down at the feet shifts the neck and trunk forward, altering the center of gravity and increasing risk, so the client should be encouraged to look straight ahead. Restraints limit mobility and lead to loss of muscle strength and are not the priority safety measure.
