The nurse acts as a witness to the client's signature and verifies that the client is competent and signing voluntarily. It is the nurse's responsibility to ensure the healthcare provider has explained the procedure and risks, and to determine if the client has remaining questions. The nurse does not explain surgical risks, as this is the provider's obligation.
