SBAR 工具包含現況（Situation）、背景（Background）、評估（Assessment）與建議（Recommendation）。重點在於針對現有病情的關鍵資訊，包含生命徵象、病史、評估結果與後續處置建議。病人幼年家族史對當下急症處置無直接幫助，非交班重點。
An effective SBAR handoff report must include the current situation with vital signs, relevant background history, assessment findings, and recommendations for ongoing care. Information such as childhood family history is not pertinent to acute clinical decision-making and should be excluded from the immediate transfer report.
