The core concept is understanding the biology of Clostridioides difficile (C. diff). C. diff produces highly resistant spores that withstand alcohol and most disinfectants. Therefore, alcohol-based hand rubs cannot kill or remove these spores. Clinically, the only effective method for physical removal is handwashing with soap and running water, using the surfactant action of soap and friction to mechanically wash spores off the skin. This is critical for preventing healthcare-associated infections (HAI), and wet handwashing must be enforced after caring for patients with diarrhea or their environment.
