DNR(Do Not Resuscitate)醫囑僅指在病人發生心跳停止(Cardiac arrest)或呼吸停止時,不進行心肺復甦術。然而,這並不代表停止所有生命照護或評估。當病人呼吸停止時,護理師仍須進行評估以確認死亡狀態,這是護理專業職責。確認後需通知醫師宣告死亡,並執行後續的臨終關懷與家屬支持。
A DNR (Do Not Resuscitate) order only specifies that cardiopulmonary resuscitation should not be performed if the patient suffers cardiac arrest or respiratory arrest. However, this does not mean that all life care or assessment is stopped. When the patient stops breathing, the nurse must still perform an assessment to confirm death, which is part of professional nursing responsibility. After confirmation, the physician must be notified to pronounce death, and subsequent end-of-life care and family support must be carried out.
台灣與美國在 DNR 執行上核心一致,皆尊重病人自主權。差異在於美國簽署 DNR 需符合州法律規範(如 POLST),且護理師在確認死亡後的行政程序與法律責任上更為嚴謹。
