The cornerstone of blood transfusion safety is the double check. The nurse must, together with another qualified nurse, verify the client's identification, the blood unit number, blood type, and cross-match results; this is the most effective line of defense against hemolytic transfusion reactions. Warming blood in a microwave damages red blood cells; infusing blood too slowly (over more than 4 hours) increases the risk of bacterial growth and hemolysis; and an overly small-gauge needle (such as 24G) causes mechanical hemolysis as red blood cells pass through.
