The first step of the nursing process is always assessment. Before transferring a client with a history of cerebrovascular accident (CVA), the nurse must determine the client's current muscle strength, balance, cooperation with commands, and presence of dizziness, in order to decide how many staff are needed and whether assistive devices (such as a transfer board or gait belt) are required. Proceeding without assessment greatly increases the risk of client falls or occupational injury to the nurse. The other steps (locking the wheels, applying a gait belt) are necessary safety measures during the transfer itself, performed after assessment based on its results.
