Never Events 是指醫療機構內本應完全預防的嚴重醫療疏失。包括開錯部位手術（A）、將病人交給未授權人士（B）、住院期間發生嚴重壓瘡（C，Stage 3+）以及手術後體內留有異物（E）。術後發燒（D）為常見併發症，非不可預防的 Never Event。
Never Events are serious medical errors within healthcare institutions that should be entirely preventable. These include wrong-site surgery (A), discharge of a patient to an unauthorized person (B), the development of a severe pressure ulcer during hospitalization (C, stage 3 or higher), and a foreign object retained in the body after surgery (E). Postoperative fever (D) is a common complication and is not a Never Event.
