Alendronate is a bisphosphonate with significant esophageal irritant potential. If the client lies down after taking the medication, the drug can reflux into the esophagus and precipitate severe esophageal ulceration or perforation. Therefore, the medication must be taken on an empty stomach in the morning with a full glass of plain water, and the client must remain upright (sitting or standing) for at least 30 minutes afterward to ensure the medication enters the stomach completely, accelerates gastric emptying, and minimizes contact with the esophageal mucosa.
