A client with schizophrenia is exhibiting signs of a delusion. Which approach is most therapeutic?
- AChallenge the reality of the client's statements
- BFocus on the feelings associated with the delusion✓
- CRedirect the conversation to a concrete topic
- DAgree with the client to maintain rapport
When working with clients with schizophrenia experiencing delusions, the core goal of nursing care is to build a trusting relationship rather than to correct their thinking. Delusions usually arise from the client's inner anxiety, helplessness, or fear, so directly challenging reality will provoke defensiveness, disrupt communication, and may even trigger aggressive behavior. Therapeutic communication should adopt an empathetic strategy, focusing on the feelings the delusion evokes rather than on the facts themselves. This approach soothes the client while allowing the nurse to maintain a professional therapeutic relationship without endorsing the content of the delusion.
