護理師作為『病人代言人』，有責任質疑不安全或不明確的醫囑。正確流程是：拒絕執行有問題的醫囑，並直接聯繫開立醫囑者進行釐清（Clarification）。若醫師堅持且護理師仍認為不妥，則應向上呈報主管，絕對不可自行執行或轉嫁他人。
As a patient advocate, the nurse has a duty to question unsafe or unclear orders. The correct process is: refuse the questionable order and directly contact the prescriber to clarify. If the physician insists and the nurse still has concerns, escalate to a supervisor. The nurse must never execute an unsafe order independently or delegate it to others.
