Digoxin has a narrow therapeutic index and toxicity occurs easily. Symptoms of toxicity include gastrointestinal disturbances (nausea, vomiting, anorexia), visual disturbances (such as yellow-green halos, diplopia), and cardiac arrhythmias. The nurse should pay particular attention to serum drug levels (toxicity is generally defined as greater than 2.0 ng/mL) and to changes in heart rate. Pulse should be assessed first; if an adult pulse is less than 60 beats per minute, the dose should be withheld and the physician notified.
