The success of discharge teaching depends on the nurse's evaluation of the patient's health literacy and self-care ability. Older adult patients often face challenges with vision, memory, or cognitive function. If the nurse fails to first assess whether the patient is able to manage medications independently, simply handing out an instructional pamphlet will be ineffective. Only after the patient's ability has been evaluated can appropriate adaptive aids (such as a pill organizer) be provided to meet their needs.
