During an acute asthma attack, bronchial smooth-muscle contraction, mucosal edema, and increased secretions raise airway resistance and produce dyspnea. The priority nursing goal is to improve oxygenation and reduce the work of breathing. The high-Fowler's position uses gravity to allow the abdominal organs to fall, creating room for the diaphragm to contract and relax more efficiently and increasing vital capacity. This is the most basic and effective position for acute respiratory distress and is consistent with the ABC (airway, breathing, circulation) principle of nursing care.
