Hyperkalemia has a dramatic effect on cardiac electrophysiology and, when severe, can produce lethal dysrhythmias. On the ECG, hyperkalemia typically evolves as follows: peaked T waves -> prolonged PR interval -> widened QRS complex -> ventricular fibrillation or asystole. Therefore, when peaked T waves are noted, the myocardium is already affected by hyperkalemia, and the physician must be notified immediately so emergency potassium-lowering measures (such as calcium, insulin with glucose) can be started.
