A nurse is preparing to administer potassium chloride intravenously to a client with hypokalemia. Which of the following is the priority assessment?
- ACheck the client's blood pressure
- BObserve for signs of peripheral edema
- CAsk about the client's preference for oral medications
- DAssess the client's hourly urine output✓
Intravenous potassium chloride (KCl) administration carries a potentially fatal risk and must be strictly controlled. Potassium is primarily excreted by the kidneys; if the client is anuric or oliguric (< 30 mL/hr), infusing potassium will rapidly lead to hyperkalemia, which can precipitate cardiac dysrhythmias and even cardiac arrest. Therefore, confirming renal function and urine output before administration is the absolute priority. The nurse should ensure an appropriate infusion rate and must never administer KCl as an IV push; it must be diluted and delivered via an infusion pump.
