Which assessment finding is most indicative of early hypovolemic shock in a client who has sustained multiple traumas?

  • ACool, clammy skin
  • BBlood pressure of 80/50 mmHg
    血壓 80/50 mmHg
  • CDecreased urine output
Explanation ·

In the early stage of hypovolemic shock, the body activates the sympathetic nervous system to maintain perfusion of vital organs, releasing epinephrine and causing tachycardia. This is the earliest physiologic sign of the compensated stage of shock. Hypotension (B) typically appears in the decompensated stage, when compensatory mechanisms fail. Cool, clammy skin (A) results from blood redistribution, and decreased urine output (C) reflects inadequate renal perfusion; both are signs of shock, but in the early stage, changes in heart rate are usually detected before changes in blood pressure or urine output.
