A nurse is preparing to administer potassium chloride IV infusion to a client. Which precautions should the nurse take? (Select all that apply.)
- AVerify that the infusion rate does not exceed 10 mEq/hr via peripheral line✓ 正解確認經周邊靜脈輸注速率不超過 10 mEq/hr
- BAssess the IV site frequently for signs of infiltration or phlebitis✓
- CAdminister the medication via IV push to ensure therapeutic levels
- DEnsure the client is producing an adequate amount of urine✓
- EDilute the medication properly according to facility protocol✓
— Explanation ·
Potassium chloride is a vesicant and must be properly diluted and administered via infusion, never as an IV push, to prevent cardiac arrest and tissue necrosis. The nurse must verify adequate urine output to ensure renal excretion and monitor the IV site for infiltration, while limiting the peripheral infusion rate to no more than 10 mEq/hr to prevent hyperkalemia.
