SBAR (Situation, Background, Assessment, Recommendation) is a standardized communication tool. A (recent vital signs and acute changes), C (medical history and current status), D (allergies and current medications), and E (recommendations and goals of care) are all key pieces of clinical communication. B (favorite TV shows) is unrelated to medical decision-making and should not appear in a formal transfer report.
