When a medication error occurs, the nurse's priorities follow the principle that patient safety comes first. The first action must be to assess the patient's physiologic response to determine whether the medication has entered the body and caused adverse effects. If the patient shows signs of allergy, abnormal heart rhythm, or altered level of consciousness, immediate emergency measures must be taken. Only after confirming the patient's safety should the nurse proceed to notify the provider, complete an incident report, and review the process. This is a typical NCLEX priority-setting question.
