病人需要灌腸。護理師應將病人置於哪種姿勢？

執行灌腸（Enema）時，病人的姿勢直接影響液體流入的效率與病人的舒適度。正確姿勢為「左側辛氏位（Left-sided Sims' position）」，即病人左側臥，右腿屈曲向腹部。這種姿勢的解剖學邏輯在於：大腸的降結腸（Descending colon）與乙狀結腸（Sigmoid colon）位於腹部左側。左側臥能利用重力原理，讓灌腸液順著腸道走向自然流入，減少阻力並能到達更深處。此外，此姿勢能放鬆肛門括約肌，降低插管時的不適與受傷風險。這在基礎護理學中是必考的標準技術規範。

When performing an enema, the client's position directly affects the efficiency of fluid instillation and the client's comfort. The correct position is the left-sided Sims' position, with the client lying on the left side and the right leg flexed toward the abdomen. The anatomic rationale is that the descending colon and sigmoid colon are located on the left side of the abdomen. Left lateral positioning uses gravity to allow the enema solution to flow naturally along the bowel, reducing resistance and reaching deeper segments. In addition, this position relaxes the anal sphincter, reducing discomfort and risk of injury during catheter insertion. This is a tested standard technique in fundamental nursing.

台美在灌腸姿勢的要求上完全一致。但在美國臨床，護理師更常使用拋棄式的「Fleet Enema」，而在台灣則常由護理佐理員或家屬協助，但護理師仍須負責正確姿勢的指導與評估。
