In clients with obsessive-compulsive disorder (OCD), ritualistic behaviors are defense mechanisms used to relieve anxiety. The focus of nursing care is response prevention — that is, gradually reducing the time or frequency of rituals while providing support, rather than abruptly stopping them. Suddenly depriving the client of their rituals provokes severe anxiety and may produce hostility toward the nurse, damaging the therapeutic relationship. The nurse must display patience and help the client redirect anxiety toward other, non-ritualistic coping strategies.
