Flushing of the face, neck, and chest during vancomycin infusion is characteristic of 'vancomycin infusion reaction' (formerly 'red man syndrome'), caused by direct histamine release when the drug is infused too rapidly. The nurse must pause the infusion, then resume at a slower rate (typically over at least 60 minutes per 1 g) after symptoms resolve, often with antihistamine pretreatment. Normal BP, mild itching without hives, and metallic taste do not require stopping the infusion.
