Anxiety is classified by severity into mild, moderate, severe, and panic levels. In moderate anxiety, the perceptual field narrows significantly; the client can attend only to the immediate stressor, with reduced awareness of surrounding details. The client at this level can still communicate and learn when guided, but concentration is impaired. The nurse should recognize this physiologic and psychological shift on assessment and use appropriate therapeutic communication techniques to stabilize the client's emotions.
