傷口癒合方式取決於組織缺損程度與癒合機制。一期癒合(Primary intention)適用於邊緣整齊、無感染的手術切口,藉由縫合或釘合使傷口迅速閉合,癒合快且疤痕極小。二期癒合(Secondary intention)則適用於組織缺損大、邊緣無法對齊、或有感染風險的傷口,無法直接縫合,必須透過肉芽組織(Granulation tissue)從傷口底部與四周逐漸填補並收縮,因此癒合過程長且疤痕較明顯。臨床上,評估傷口癒合關鍵在於監測感染徵象(如紅腫熱痛、膿性分泌物),因二期癒合傷口對外部環境更為脆弱。

Wound healing depends on the degree of tissue loss and the healing mechanism. Primary intention applies to clean, well-approximated surgical incisions without infection, where rapid closure is achieved through sutures or staples, resulting in fast healing and minimal scarring. Secondary intention applies to wounds with significant tissue loss, non-approximated edges, or risk of infection that cannot be closed directly; healing occurs as granulation tissue gradually fills the wound bed from below and the wound contracts, resulting in a longer healing process and a more prominent scar. Clinically, monitoring secondary-intention wounds for signs of infection (redness, swelling, heat, pain, purulent drainage) is essential because they are more vulnerable to environmental exposure.
