Following thyroidectomy, the most lethal complication is airway obstruction. Because the surgical site lies adjacent to the trachea, postoperative bleeding (hematoma) or tissue edema may rapidly compress the airway and result in asphyxiation. According to the ABC (Airway, Breathing, Circulation) principle, maintaining a patent airway is always the highest priority. Other complications such as hypocalcemia or infection are also important but do not typically cause life-threatening compromise within minutes; therefore, airway assessment and monitoring are the nurse's foremost task.
