「哨兵事件」（Sentinel Events）是指在醫療機構中發生的非預期性事件，導致病人死亡、永久性傷害或嚴重的暫時性傷害。這類事件要求機構進行根本原因分析（Root Cause Analysis, RCA），以識別系統性缺失並預防再犯。判斷標準在於事件是否造成了重大且不可逆的傷害，或嚴重危及病人安全。
Sentinel events are unexpected occurrences in healthcare causing death, permanent harm, or severe temporary harm. These require institutions to perform Root Cause Analysis (RCA) to identify systemic gaps and prevent recurrence. Criteria are whether the event caused major irreversible harm or severely endangered patient safety.
