The most important priority after total hip replacement (THR) is the prevention of dislocation. The affected side should be maintained in abduction, typically using an abduction pillow or by placing a pillow between the legs. Adduction (C), hip flexion greater than 90 degrees (D), and internal/external rotation (A) must be avoided because these positions increase the risk of the prosthesis dislocating from the acetabulum. The patient must be taught not to cross the legs or to bend forward excessively to put on shoes.
