副甲狀腺切除術後,最嚴重的潛在併發症是低血鈣(hypocalcemia)。副甲狀腺素(PTH)負責調節體內鈣離子濃度,手術過程中若損傷副甲狀腺,會導致 PTH 分泌不足,引起血鈣急速下降。低血鈣會增加神經肌肉的興奮性,出現口周麻木、刺痛(Trousseau's 或 Chvostek's sign),嚴重時可導致喉頭痙攣(laryngospasm)甚至呼吸停止。

The most serious potential complication after parathyroidectomy is hypocalcemia. Parathyroid hormone (PTH) regulates calcium; surgical injury to the parathyroid glands reduces PTH and produces a rapid drop in serum calcium. Hypocalcemia increases neuromuscular excitability, causing perioral numbness and tingling (positive Trousseau's or Chvostek's sign) and, in severe cases, laryngospasm and respiratory arrest.
