Blood transfusion is a high-risk procedure. Nursing responsibilities include performing a two-nurse verification (bedside check of the patient, blood bag, and blood type), using a filter, administering only with 0.9% normal saline (to avoid hemolysis), and closely monitoring vital signs during the first 15 minutes for early detection of acute hemolytic reactions. Blood must never be warmed using a microwave, as this denatures proteins and causes hemolysis.
