After transurethral resection of the prostate (TURP), the purpose of continuous bladder irrigation (CBI) is to flush out clots and prevent obstruction of the urinary catheter. If the drainage appears bright red with large clots, this means bleeding is occurring faster than the irrigation can clear it, or clots are beginning to obstruct the catheter. The nurse should immediately increase the rate of irrigation to disperse the clots and prevent catheter obstruction, bladder spasm, and distention. This is the most common postoperative nursing priority.
