美國護理站常有分級制度，資深護理師（Charge Nurse 或 Preceptor）通常負責病情最複雜（High acuity）的病人。與台灣不同點在於，美國護理師對「病情惡化（Clinical deterioration）」的自主判斷門檻較高，需依據 SBAR 格式即時向醫師匯報。
